What Information do I Include in ARC Form 56-R?
There are three sections in this form. Section 1 is devoted to the personnel officer’s data. An applicant must provide the following information in this part: full name, last four hours of the social security number, branch/primary AOC, current grade, current organization (CPU, IMA, IRR or AGR). Section 2 is called the Review by Officer’s Immediate Commander. The information in this part concerns the officer’s position. The following details must be provided: authorized grade, branch, unit of assignment, UIC, position number, report date, date of mobilization, etc. You must indicate the grade and the name of representative, unit name, email address and signature in the last part of the form.
